Registration

• Pre-registered members (non-US citizens) – $15.00
  Must register no later than July 27.
  
  
  Must provide to Peter Kowalski (pkowalski@cdc.gov) the following information:
  Gender
  Date of birth
  Place of Birth (City province, state, country)
  County of Citizenship
  Passport Type and Number
  Date of Passport Issuance
  Type of visa and expiration (if visitor is Permanent Resident – resident #)
• Pre-registered Members (US Citizens) – $15.00
  Must register no later than 5 days prior to meeting
• Pre-registration - non-Members (non-US citizens) – $20.00
  Must pre-register no later than July 27.
